Description
Provides training and support to members with disabilities to assist and enable them to obtain and maintain long-term, competitively paid employment. Works in collaboration with agency staff, members, families, and other service providers to promote the full inclusion of people with disabilities in their communities. Report directly to the Employment Services Program Manager. Duties require the use of discretion and independent judgment.
Requirements
Education:
Undergraduate degree in human services, psychology or related field preferred, but not required.
Experience:
• One year experience in direct-services with people with disabilities required.
• Ability to develop, implement, and monitor goal plans.
• Ability to relate well to a variety of individuals, employers, and community groups, work effectively with a multidisciplinary team, define work priorities and work independently with minimal supervision.
• Knowledge of Individualized Person-Centered Service Plan (which may include job development activities, on-the-job training requirements and community resources for job strategies.
• Understand how to analyze job factors to determine job skill requirements, performance expectations of employer and job coaching/support needs for on-the-job training.
• Maintain and sumits written progress notes for each Service.
• Completes all required documentation in a timely manner.
• Provide follow-up support and monitor members work performance progess through regular contacts with employer and member.
• Provide necessary interventions to support the members in maintaning employment and attaining their stated goals.
